Leaving the hospital is often seen as the end of a medical crisis, but for many seniors, it marks the beginning of a new and fragile stage of recovery. Acute care can stabilize an illness, treat an injury or support a surgery. Still, it does not always restore a person’s strength, balance, confidence and ability to manage daily life. Older adults are often discharged while still coping with weakness, pain medication changes, reduced mobility or confusion about what comes next. Without the right support, this period can be difficult and even risky.
