The program addresses the most vulnerable points in elder care: the transition from hospital to home or community living. It follows clients from discharge planning through their transition, ensuring continuity when they’re most at risk of falling through the system cracks.
When a client is hospitalized, the team tracks their progress, keeps family members informed and ensures that critical details are not lost in rushed discharge processes. These details include new medications, updated doctor’s orders or changes in mobility. Without this advocacy presence, a poorly managed hospital discharge can undo weeks of progress and destabilize a person’s ability to remain in their chosen living situation.
